Ministry Reporting Requirements - Surgical Safety Checklist

What are hospitals reporting on their website?

Beginning July 30, 2010, each eligible hospital will post on its website, by site, the percentage compliance where all three phases of the Surgical Safety Checklist (SSCL) were performed for all surgical procedures in that period.

What is the Case Definition for the Surgical Safety Checklist?

The Safe Surgical Checklist compliance indicator is a process measure, and refers to the percentage of surgeries in which a three-phase surgical safety checklist was performed correctly and appropriately for each surgical patient. All surgeries carried out in a fully equipped operating room are eligible to report the use the SSCL.

The percent compliance is calculated as follows:

Number of times all three phases of the Surgical Safety Checklist was Performed) divided by (Total Surgeries) Times 100 = Percentage
